Symantec (formerly Veritas) have issued a technical advisory for Veritas NetBackup.
The following is an extract from the advisory:

"This critical technical issue only affects the following products:

- -VERITAS NetBackup (tm) Enterprise Server 5.1 Maintenance Pack 3 (MP3)

- -VERITAS NetBackup (tm) Server 5.1 Maintenance Pack 3 (MP3)

Due to a possible corruption issue (under very limited circumstances)
with NetBackup (tm) Enterprise Server/Server 5.1 MP3, the 5.1 MP3 Server
patches have been pulled from the Support site (NetBackup(tm) Database
Agents, Options and Japanese 5.1 MP3 patches are not affected). For a
detailed description of this issue and our recommendations, please
review the following reference document"
